Claims facilitation ![]() Consolaré will work with you to ease the frustration and mountain of paper that insurance can create. While our organization does not accept insurance, many of our patients prefer to combine Consolaré services with existing medical care options. After each visit you will receive a completed insurance claim form (HCFA) for all services you received that day. Your only responsibility is to sign the form and put it in the mail – we even provide the stamped envelope addressed to your insurance company. Most insurance plans will reimburse you a percentage of these itemized charges; however, some of the preventive services we provide are not reimbursable under current health plans. Consolaré provides no coverage for prescription medications or any other health expenses you might incur outside our office. At Consolaré, there is never a co-pay or deductible to meet. All of your healthcare coverage is inclusive in your annual fee as outlined in each of our Life Plans. However, health insurance is required when you need to visit a specialist, check into a hospital, or access other healthcare services. Remember, that in addition to having a portion of your services reimbursed, your annual fee may also be tax-deductible but please consult your tax advisor. In some cases the fee is reimbursable under your company’s Medical Savings Plan, Health Savings Account (HSA), 125, or Cafeteria Plan. You need to contact your Human Resources Department to see if part, or all, of this fee is reimbursable under your existing plan.
