Annual Membership includes UNLIMITED and DIRECT access to your physician 24/7/365 via telephone or home / office visits with NO additional costs. How amazing would it be to see YOUR physician, on YOUR schedule - whenever you want?

Having a PERSONAL physician on-call 24/7 encourages you to be responsive to your healthcare needs, not to put them off and hope you eventually get better.

Membership fee is $6,000 annually and each additional family member will receive a 25% discount.

Children ages 12-18 receive special pricing at $1500/year with adult membership.

If you are not completely satisfied with the value and service of Consolaré I will refund your entire membership fee.

In addition to the highest level of care our Livello Primo Plan includes the following diagnostic workup:

Annual Comprehensive Physical Exam, Medical History, and Health Risk Analysis
Electrocardiogram
Prostate or Gynecological exam
Unlimited sick visits
One on one/family counseling and risk assessment
Prevention and education
Nutrition and weight loss packages available upon request and discounted packages available
Pre-travel examinations and vaccinations (if required)
Breast exams and breast cancer screening
Body Mass Index assessment
