Because our health plans are custom tailored to fit specific needs and goals we offer three unique Life Plan options along with several Customizable Care Options that can be added to your desired plan to accommodate the most sophisticated needs. Since you pay an annual fee for our services, there are no co-pays or additional charges for services rendered by our physicians. Our preventive approach provides diagnostic and imaging services that are typically not medically reimbursable by your insurance provider. In other words, you and your family not only receive access to care but additional state-of-the-art medical information about your health that normally goes undiagnosed. All testing and diagnostic work outlined below is done to establish a unique baseline for each of our clients. Your Consolaré physician will then review and explain all test results with you, and make lifestyle recommendations in order for you to attain your healthcare goals. Based upon your lifestyle objectives, medical history, and current health status additional testing and diagnostic work will be completed and reviewed on an as-needed basis. In addition to state-of-the-art care, each Consolaré patient has online access to his or her medical portfolio. This includes medical records, summaries of visits, physician recommendations, and other applicable information.
